Warren High School and Warren Middle School are implementing an electronic alternative for student-athletes, called Privit, for managing pre-participation physical exams and emergency medical cards. This system will be taking the place of paper-based forms Warren has previously used.
This will allow parents and student athletes to complete a one-time registration where the full medical history is completed. After the initial set-up, any changes in medical history can be updated as needed. Because Privit is web-based, if a student athlete were to become injured in an emergent situation, hospitals or emergency departments would be able to, with a provided code, access the full medical history. This allows higher quality care to be delivered.
All parents/guardians of student athletes will be required to complete this for each athlete and can do so from home by accessing the website below:
Any parent of a fall sports athlete who does not complete this at home will have the opportunity to do so at the High School at 5pm on Wednesday, July 26th before the Mandatory OHSAA Fall Sports Meeting.
